SUMMARY:SITE VISIT IN SEVILLE - Understanding the implementation of the Multimorbidity Care Model at a primary care centre
DESCRIPTION:Context: On November 20 and 21\, two important meetings were organised by CHRODIS PLUS at the Andalusian Health Service (SAS) headquarters in Seville: a workshop by Work Package 4 – Integration to National Policies and Sustainability\, and then the Executive Board meeting. After the Executive Board meeting\, members of the EB visited a primary health centre in Seville. \nSite visit to the Primary Healthcare Centre Las Palmeritas \nThe aim of the site visit to the Primary Healthcare Centre Las Palmeritas was to provide a better understanding the selected Andalusian approach for the pilot implementation within the Work Package 6 at regional level. Work Package 6 focuses on the implementation of Integrated Care Model for Multimorbidity at 5 pilot sites in Europe\, including Seville. \nThe Work Package 6 pilot implementation in Andalusia addresses component-4 (individualized care plans) of the Integrated Care Model for Multimorbidity-ICMM developed during the former Joint Action Chrodis. The general purpose of the intervention in the pilot is the assessment of the influence of the systematized application of individualized and comprehensive care plans to complex chronic patients\, within the framework of the Andalusian Public Health System strategies at primary healthcare centres. \nDuring the site visit\, first a general overview of the Andalusian Public Health System-APHS was provided by the Regional Ministry of Health of Andalusia (CSJA) project coordinator (Dr Ana Carriazo) Then the Director of the Primary Health Centre (Dr Inmaculada Gabaldón) presented how the Integrated Care Process ‘Healthcare for Multimorbidity Patients’ works in practice at the centre. She also analysed how this work is in line with the Andalusian comprehensive health plans and strategies: mainly the ‘Comprehensive Healthcare Plan for Patients with Chronic Diseases’\, the Andalusian ‘Comprehensive Care Strategy’ and the ‘Andalusian Plan for Primary Healthcare Renewal’. A fruitful debate among participants to clarify the particularities of the Andalusian model was also organised.
