SUMMARY:SUNFRAIL seminar on frailty and multimorbidity
DESCRIPTION:The SUNFRAIL project organises a seminar to share preliminary results achieved throughout the first two years of the projec. The seminar will be held in Brussels on 5 April 2017. \nSUNFRAIL (SUNFRAIL Reference Sites Network for Prevention and Care of Frailty and Chronic Conditions in community dwelling persons of EU Countries) is a European project with a duration of 30 months\, which started in May 2015. The project receives funding from the EU Health Programme 2014-2020 and brings together 11 partners from 6 EU Member States. \nDuring the event\, you will have an opportunity to familiarise yourself with the SUNFRAIL Tool for the identification of frailty and multimorbidity\, and to hear more about good practices for the identification of frailty and multimorbidity collected by the project partners.
