SUMMARY:JA-CHRODIS Health Promotion Webinar
DESCRIPTION:[vc_row][vc_column][vc_column_text disable_pattern=”true” align=”left” margin_bottom=”0″]\nJA-CHRODIS Health Promotion: Good practices and transferability in the field of health promotion and chronic diseases prevention across the life cycle\nThe second JA-CHRODIS webinar took place on Tuesday\, 11th of October\, from 14-15h (CET). \nAlexander Haarmann from the German Federal Center for Health Education (BZgA)\, task leader of the Health Promotion and Disease Prevention work package\, provided an overview of the outcomes & results achieved. \nBackground\nMany chronic diseases are preventable\, or their onset can be delayed\, through policies and interventions that address modifiable individual and social risk factors. Yet only 3% of health costs in EU Member States are presently invested in prevention measures\, whereas about 97% are spent on treatment. \nThe EU Reflection Process on Chronic Diseases (September 2013) identified the potential that lies in systematically mapping\, validating and transferring good practices that exist across the EU in relation to health promotion and primary prevention of chronic disease. \nThe Health Promotion work package worked to develop this potential\, focusing on cardiovascular diseases\, stroke and type 2 diabetes. The work has taken into account lifestyles and health-related behaviour as well as the socioeconomic determinants that influence them. \nThe recording of the webinar is available on our YouTube channel: https://www.youtube.com/watch?v=up8xf28gZZ4 \nDownload the webinar presentation here.[/vc_column_text][/vc_column][/vc_row]

SUMMARY:JA-CHRODIS Multimorbidity Webinar
DESCRIPTION:[vc_row][vc_column][vc_column_text disable_pattern=”true” align=”left” margin_bottom=”0″]\nDeveloping a care model for multimorbid patients\nThe third of JA-CHRODIS Webinar series focused on the work of the Multimorbidity work package and took place on Tuesday\, 18th of October\, from 14-15h (CET). \nBackground\nLife expectancy in the EU is higher than ever before\, due in part to good and improving healthcare. However\, the co-occurrence of multiple chronic conditions in one person\, i.e. multimorbidity\, has become increasingly common as well. 65% of people over 65 are affected by multimorbidity. This number rises to 85% for the 85-year-old group. \nComplex healthcare needs of multimorbid patients require the involvement of a large number of healthcare providers and a vast amount of resources. Often patient care is fragmented\, expensive and fails to respond to the needs of the patient. While there are examples of comprehensive care programmes being implemented in relatively small populations\, there are no widely accepted care models available across the EU Member States\, due to organisational differences. \nThe Multimorbidity work package reviewed existing patient-centred comprehensive care programmes in order to identify the needs of the participating countries’ healthcare systems and developed a new comprehensive care model that foresees improved care and coordination and better support of patients’ self-empowerment. \nDownload the Webinar’s presentation \nThe recording of the webinar is available on our YouTube channel: https://www.youtube.com/watch?v=spmJCOVl6cw[/vc_column_text][/vc_column][/vc_row]

SUMMARY:JA-CHRODIS Diabetes Webinar
DESCRIPTION:[vc_row][vc_column][vc_column_text disable_pattern=”true” align=”left” margin_bottom=”0″]\nAddressing the burden that diabetes places on individuals and health systems\nThe fourth JA-CHRODIS webinar will focus on the results of the Diabetes work package and will take place on Tuesday\, 25th of October\, from 14-15h (CET). \nBackground\n32 million EU adults (8.2%) had diabetes in 2013\, and this number could rise to 38 million by 2035. Diabetes is the fourth most common cause of death in Europe. \nThe effective treatment and management of diabetes calls for comprehensive approaches to care that empower patients and enable them to self-manage their disease to the best possible extent.  There are good models across the EU on the prevention and management of type 2 diabetes\, that can serve as an example to others and that can also apply to other chronic diseases. \nThe Diabetes work package addresses the challenge that the burden of diabetes places on health systems and individuals. Its principal objective is to improve coordination and cooperation among Member States to act on diabetes\, including the exchange of good practices across the EU. Special emphasis is given to support the development and implementation of National Diabetes Plans. \nThe recording of the webinar is available on our YouTube channel: https://www.youtube.com/watch?v=fm0auwanTyY \nDownload the webinar’s presentation here.[/vc_column_text][/vc_column][/vc_row]
