DESCRIPTION:JA-CHRODIS was present at the 18th European Health Forum Gastein\, a three-day event held on 30 September to 2 October 2015 in Austria. The Forum is one of the most important health-related event in the European Union and a meeting point for experts from different fields within the health system. Co-leader of the multimorbidity work\, Rokas Navickas contributed to the session Facing Multimorbidity: Challenges and Responses on Day 2\, sharing essential facts and key trends of co-morbidity across Europe. DESCRIPTION:4th Meeting of work package 7\nOctober 20 – 21\, 2015 @ 9:00 – 16:00 • Ljubljana\, Slovenia\n \nNational Institute of Public Health of the Republic of Slovenia as a co-leader of the work package 7 of  the JA-CHRODIS organised the 4th meeting of the work package 7\, Diabetes: A Case Study on Strengthening Health Care for People with Chronic Diseases. \nThe objectives of this meeting were to make an update on activities of the WP7 and to present\, discuss\, and get the input from the Partners of WP7 and other participants. SUMMARY:Multimorbidity Expert Meeting
DESCRIPTION:The main achievement during 2015 for the JA-CHRODIS work package on multimorbidity was the development of a framework for care of multimorbidity patients that can be applied across Europe. \nFirst\, we identified a list of 20 components across five domains from existing published comprehensive care programs for patients with multiple chronic conditions or frailty. An expert meeting was held in Brussels on October 28\, to discuss the components. Sixteen were selected after discussion. During the meeting the experts discussed the definition of the components\, aims\, key characteristics\, target population and relevance for patients with multimorbidity in order to develop a framework for care of multimorbidity patients\, including recommendations for delivery of system design\, decision support\, self management support\, clinical information systems\, and community resources. \nAn article has been produced to detail the results and conclusions of this meeting\, and has been reviewed\, edited and\, coauthored by all the experts\, entitled “Multimorbidity care model: Recommendations from the consensus meeting of the Joint Action on Chronic Diseases (CHRODIS)”. \nDownloads\nMinutes
