SUMMARY:Closing the evidence-practice gap on NCDs: Translation\, transfer and sustainability for comprehensive responses across Europe
DESCRIPTION:The session is part of the online conference ‘EHFG 2020: Dancing with elephants: New partnerships for health\, democracy\, business’ organized by the European Health Forum Gastein. \n \nOn the surface\, the steps to closing the gap between research findings and existing clinical practice are simple: 1. Identifying good practice; 2. Piloting; 3. Scaling-up. However\, promising initiatives – and the big players overseeing them – often get stuck at the first step. At the same time\, mechanisms like crowdfunding and startups have proven to mark a shift from the few major players towards community driven initiatives\, often based on ideas and passion. \nThis session will showcase how ideas and passion on a small scale can be transferred to cross-national learning while respecting national interests\, and explore the success factors to piloting\, transferring and sustaining good practices that benefit patients affected by chronic disease. We will showcase outcomes of the CHRODIS+ Joint Action\, focused on supporting Member States in scaling up practices aimed at reducing the burden of non-communicable diseases\, and discuss how learnings on both the successes and barriers from the Joint Action can be taken forward to help health stakeholders achieve policy impact in the future. Join us to be guided through the potential dancing steps of ambitious partnerships in the battle against chronic disease!

DESCRIPTION:ON BEHALF OF JOINT ACTION CHRODIS PLUS\n WE WOULD LIKE TO THANK YOU FOR YOUR PARTICIPATION AT THE CONFERENCE\nKey conference materials will be made available soon here. \n[table id=30 /]\nPRESS RELEASE\nOver its 3-year lifespan\, the aim of CHRODIS PLUS Joint Action (2017-2020) was to support Member States in overcoming the burden of chronic diseases by promoting policies and practices that have proved successful in the past. Further refinement and EU cross-border sharing of these tested policies and good practices has been the core idea behind this pan-European project. \nCHRODIS PLUS has implemented 21 pilot projects aimed at testing tools and good practices for certain chronic diseases\, and held 17 policy dialogues (15 national and 2 EU-level ones). It has brought together over 50 partners from 21 European countries. \nAll good things come to an end: CHRODIS PLUS held its closing online conference on 27 October 2020. \nThis conference illustrated the impact that CHRODIS PLUS has had on public health systems across EU member states over the past 3 years. It focused on showing how the good practices\, models and tools implemented by the project can be tailored to various national and local settings across Europe. \nThe conference’s presentations and roundtable discussions were structured around the project’s key focus areas: \n\nHealth Promotion & Primary Prevention\nAn Integrated Multimorbidity Care Model\nFostering Quality Care for People with Chronic Diseases\nICT-based Patient Empowerment\nEmployment & Chronic Diseases
