DESCRIPTION:Vilnius University Hospital Santariškių Klinikos\, co-leader of the Joint Action’s work on multimorbidity\, organised the conference “Networks of Health Institutions – the Best Response to the European Regional Policy”\, which took place on the 26th of January in Vilnius\, Lithuania. SUMMARY:SUNFRAIL seminar on frailty and multimorbidity
DESCRIPTION:The SUNFRAIL project organises a seminar to share preliminary results achieved throughout the first two years of the projec. The seminar will be held in Brussels on 5 April 2017. \nSUNFRAIL (SUNFRAIL Reference Sites Network for Prevention and Care of Frailty and Chronic Conditions in community dwelling persons of EU Countries) is a European project with a duration of 30 months\, which started in May 2015. SUMMARY:Pre-implementation workshop of Work Package 7 in Ljubljana
DESCRIPTION:The workshop was held on 4-5 June in order to provide the partners with pilot actions operational support for developing the Pilot Action Plan based on the Guideline for the pre–Implementation phase\, and instructions on how to use the Quality Criteria Recommendation Tool. Representatives from Task 2 (Task 2: Piloting the Quality Criteria and Recommendations Tool through pilot actions across different countries) pilot sites (Slovenia\, Croatia\, Serbia\, Greece\, and Finland) were present. \n  \nThe pilot action in Slovenia was used as a case study to verify all the necessary steps to define the  Pilot Plan. The two-day workshop mostly focused on group work in a preparatory process. At the same time\, the experience served as field training for sharing the knowledge\, skills and tools necessary for carrying out the SWOT analysis and the design of the Pilot plan within a local context. \n

The presence of patients who are using the application underlined the added value and strengthen the importance of new technologies in the service of health. SUMMARY:JOGG site visit in Helsinki
DESCRIPTION:The event of site visit organized under the auspices of Joint Action Chrodis+ was held on August 13-14th in Helsinki\, Finland\, hosted by THL.  The event provided a platform for deeper approach understanding of the methodology  of JOGG implementation both at local and national level. Selected elements of JOGG will be adapted and implemented within the Health Promoting Community Programme (HPC) in Iceland by the Directorate of Health. \nThe  JA-CHRODIS Good Practice “JOGG” addresses children and adolescent obesity and encourages all people in a town or neighbourhood to make healthy food and physical activity an easy and attractive lifestyle option for young people. HPC is an umbrella for comprehensive health promotion work in municipalities\, including all school levels. HPCs main aim is to support communities/schools to create supportive environments that promote healthy lifestyle\, health and well-being for all. SUMMARY:SITE VISIT IN SEVILLE - Understanding the implementation of the Multimorbidity Care Model at a primary care centre
DESCRIPTION:Context: On November 20 and 21\, two important meetings were organised by CHRODIS PLUS at the Andalusian Health Service (SAS) headquarters in Seville: a workshop by Work Package 4 – Integration to National Policies and Sustainability\, and then the Executive Board meeting. After the Executive Board meeting\, members of the EB visited a primary health centre in Seville. \nSite visit to the Primary Healthcare Centre Las Palmeritas \nThe aim of the site visit to the Primary Healthcare Centre Las Palmeritas was to provide a better understanding the selected Andalusian approach for the pilot implementation within the Work Package 6 at regional level. Work Package 6 focuses on the implementation of Integrated Care Model for Multimorbidity at 5 pilot sites in Europe\, including Seville. \nThe Work Package 6 pilot implementation in Andalusia addresses component-4 (individualized care plans) of the Integrated Care Model for Multimorbidity-ICMM developed during the former Joint Action Chrodis. The general purpose of the intervention in the pilot is the assessment of the influence of the systematized application of individualized and comprehensive care plans to complex chronic patients\, within the framework of the Andalusian Public Health System strategies at primary healthcare centres. \nDuring the site visit\, first a general overview of the Andalusian Public Health System-APHS was provided by the Regional Ministry of Health of Andalusia (CSJA) project coordinator (Dr Ana Carriazo) Then the Director of the Primary Health Centre (Dr Inmaculada Gabaldón) presented how the Integrated Care Process ‘Healthcare for Multimorbidity Patients’ works in practice at the centre. She also analysed how this work is in line with the Andalusian comprehensive health plans and strategies: mainly the ‘Comprehensive Healthcare Plan for Patients with Chronic Diseases’\, the Andalusian ‘Comprehensive Care Strategy’ and the ‘Andalusian Plan for Primary Healthcare Renewal’. A fruitful debate among participants to clarify the particularities of the Andalusian model was also organised. DESCRIPTION:Focus of the event: Ensuring that implementation projects meet patients’ expectations\, empower the target population and train to promote empowerment \nEvent organised by the European Patients’ Forum \nThe workshop took place on December 3rd – 4th 2018 in Belgrade\, Serbia \nObjective of this workshop was to set standardized pathways for the implementers of the Quality Criteria Recommendations Tool and to prepare them to successfully run the study visits that will take place in 5 implementing countries in 2019. Study visits are an important step in the implementation process. This excercise preceded the finalization of Pilot Action Plan as the essential tool for future operational planning and monitoring. \nAn important step of the implementation process is a site visit at these 8 locations during the first half of 2019 by a group of experts including the leaders of this work package. \nDuring this workshop\, the details of these site visits were planned thoroughly. The study visits were designed from a patient/person perspective to assess if the pilot activities meet patients’/persons’ expectations with a special emphasis given to the respective QCR Tool\, on empowering the target population\, as well as education and training to promote empowerment. \nValentina Strammiello\, Senior Programme Manager from  EPF commented on the event: “The WP7 Workshop was of key importance for the implementation phase of CHRODIS + project in general and specifically for the work undertaken in WP7. SUMMARY:Policy dialogue in Portugal on the Advertisement of Food and Beverages to Children
DESCRIPTION:A Policy Dialogue was held\, on the 30th of January\, at the Directorate-General of Health of Portugal. This meeting was focused on the Advertisement of Food and Beverages to Children. The main objective of this Policy Dialogue was to gather a group of national stakeholders\, in order to find ways to tackle this issue at a national level. \n\nIn this Policy Dialogue it was possible to shed a light on what each entity has done regarding this issue; to get to know what areas each entity considers more relevant\, within this issue and to discuss the most effective way of regulation – regulation vs. self-regulation. \n\nThere was also an identification of the main barriers regarding the regulation of Advertisement of Food and Beverages to Children. Additionally\, each entity stated their availability to develop new actions to tackle this issue in an intersectoral way. Afterwards\, there was a debate to define a strategy towards the conclusion of the legislative process on the Portuguese Advertising Code that is already ongoing.  \nThe Portuguese policy dialogue fits neatly with the Chrodis+ aim of adopting health promotion and primary prevention strategies as the best way to reduce the burden of chronic diseases. In this case it is through improving the environment that children and young people grow up in by maximising their opportunities to live long\, healthy lives by focussing on health dietary practices.\nAll entities present in this Policy Dialogue were directly or indirectly working on issues related to this subject. In general\, all entities agree with self-regulation but consider that it should coexist with regulation. It was said that the role of Health Literacy may be preponderant in changing behavior amongst children\, young people and families. Regarding perceived barriers\, digital media brings new challenges for the regulation of advertised content. Furthermore\, the measures to be applied must focus not only on the media and digital platforms\, but also on actions that attract children and young people\, such as festivals\, merchandising\, sporting events\, among others. It will be important to use digital influencers as partners for promoting healthier food choices and the “counter advertising” method may be a very important debunk to demystify myths about traditional advertising. \n\nThe next steps for Portugal will be to create a working group with the main stakeholders for the conclusion of the alteration of the Portuguese Advertising Code; and\, regarding communication\, to define several actions of Health Literacy for children\, youth and families\, through positive messages. \n[Best_Wordpress_Gallery id=”24″ gal_title=”Portugal Policy Dialogue_30 Jan 2019″]

The aim is to generate practical and policy level lessons to support Member States in tackling the burden of chronic diseases. During its 36 months of operation\, this project contributes to the reduction of this burden by promoting the implementation of policies and practices that have been already successfully tested. The further refinement and cross-border sharing of these tested policies and good practices across EU countries is the core idea behind this action. \nThe CHRODIS PLUS Budapest Conference on 14-15 May has been one of the key events in this project where over 180 chronic disease experts arrived from 26 countries to discuss and provide feedback on the real-life experiences project partners have gained during the initiative’s first 20 months. This conference demonstrated the impact that CHRODIS PLUS has on public health systems across EU Member States with a special emphasis on proving that the good practices\, models and tools that the project implements can be tailored to various national and local settings across Europe. Alternatives for effective implementation of Article 35 of the Spanish Public Health Act 33/2011 \n  \nExtended explanation:  \nThis policy dialogue focused on Health Impact Assessment (HIA). HIA is a useful tool for evaluating health policies but also for increasing awareness about the Health in All Policies approach and for boosting inter-sectoral collaboration. \nThe goal of this policy dialogue was to consider the integration of HIA into environmental projects and policies\, thus enhancing collaboration between the Ministry of Health\, Consumer Affairs and Social Welfare and the Ministry for the Ecological Transition. \n  \nGeneral Intro to CHRODIS PLUS Policy Dialogues: \nThis is the summary of one of the 17 policy dialogues organised under the Chrodis PLUS Joint Action. For a summary of all Policy dialogues please visit the chrodis.eu/policy-dialogues webpage. These dialogues are to serve as inspiration for policy makers in other countries who are seeking good practices to consider when planning their own national policy dialogue processes. \n  \nProblem identification: \nThe specific question for this policy dialogue was to elucidate which would be the most effective way to implement in practice HIA in all Spanish Regions and therefore developing Article 35 of the Spanish Public Health Act 33/2011 that mandates that selected regulations\, plans\, programmes and interventions with an impact on health are subjected to HIA. \n  \nWhat is the primary reason for choosing this topic in your country? \nArticle 35 of the Spanish Public Health Act 33/2011 does not specify which policies are eligible for HIA. Additionally\, underway of finished policies are not eligible for HIA. However\, their evaluation would provide useful information to policy makers. \nFurthermore\, the Act does not mandate to specifically use HIA methodology (or whether other methodologies are eligible).   HIA might be useful for evaluating some policies but others are not subjected to HIA. Other useful methodologies should be taken into account. \nEnvironmental Impact Assessment (EIA) was already underway and mandatory. A debate among experts about integrating HIA into EIA as an effective solution is underway and some Spanish Regions are already implementing this approach. \n  \nThe policy dialogue process: \nA high level meeting with the participation of the Secretaries of both Ministries took place. The main dialogue was held between the General Director of Public Health and the General Director of Environmental Quality. After introductions by the representative from EuroHealthNet\, and the other two keynote speakers from the General Directorate of Public Health\, Quality and Innovation\, the dialogue answered to the following questions: \n·       Health and Environment are intimately related factors. The primary care centre Actur-Norte and the Chronic Care Unit at Royo Villanova Hospital were both visited. The implementation team presented the three main strategies of the practice: training of professionals in multimorbidity (eMultiPAP)\, integrating complex chronic patients components (comprehensive assessment\, virtual interconsultation\, and electronic health records)\, and community care actions (health needs identification and resources map). The meeting closed with a debate on the facilitators\, barriers and benefits of the program.  \n \n

It provides information about the birth of the CHRODIS PLUS Training Tool\, which has been developed to help employers understand the benefits of the workplace inclusion\, integration and reintegration of people suffering from chronic diseases. Managers are encouraged to implement the Training Tool to evaluate inclusiveness and work ability of their teams so as to create facilitating environments for reaching desirable organizational outcomes: productivity\, job satisfaction of all employees\, and organizational commitment.
