Yet only 3% of health costs in EU Member States are presently invested in prevention measures\, whereas about 97% are spent on treatment. \nThe EU Reflection Process on Chronic Diseases (September 2013) identified the potential that lies in systematically mapping\, validating and transferring good practices that exist across the EU in relation to health promotion and primary prevention of chronic disease. \nThe Health Promotion work package worked to develop this potential\, focusing on cardiovascular diseases\, stroke and type 2 diabetes. DESCRIPTION:[vc_row][vc_column][vc_column_text disable_pattern=”true” align=”left” margin_bottom=”0″]\nDeveloping a care model for multimorbid patients\nThe third of JA-CHRODIS Webinar series focused on the work of the Multimorbidity work package and took place on Tuesday\, 18th of October\, from 14-15h (CET). \nBackground\nLife expectancy in the EU is higher than ever before\, due in part to good and improving healthcare. However\, the co-occurrence of multiple chronic conditions in one person\, i.e. multimorbidity\, has become increasingly common as well. 65% of people over 65 are affected by multimorbidity. This number rises to 85% for the 85-year-old group. \nComplex healthcare needs of multimorbid patients require the involvement of a large number of healthcare providers and a vast amount of resources. Often patient care is fragmented\, expensive and fails to respond to the needs of the patient. Diabetes is the fourth most common cause of death in Europe. \nThe effective treatment and management of diabetes calls for comprehensive approaches to care that empower patients and enable them to self-manage their disease to the best possible extent.  There are good models across the EU on the prevention and management of type 2 diabetes\, that can serve as an example to others and that can also apply to other chronic diseases. \nThe Diabetes work package addresses the challenge that the burden of diabetes places on health systems and individuals. Its principal objective is to improve coordination and cooperation among Member States to act on diabetes\, including the exchange of good practices across the EU. Representatives from Task 2 (Task 2: Piloting the Quality Criteria and Recommendations Tool through pilot actions across different countries) pilot sites (Slovenia\, Croatia\, Serbia\, Greece\, and Finland) were present. \n  \nThe pilot action in Slovenia was used as a case study to verify all the necessary steps to define the  Pilot Plan. The two-day workshop mostly focused on group work in a preparatory process. At the same time\, the experience served as field training for sharing the knowledge\, skills and tools necessary for carrying out the SWOT analysis and the design of the Pilot plan within a local context. \n

SUMMARY:Site visit - mHealth tools for chronic diseases
DESCRIPTION:We had a privilage to participate at the presentation of our partner organization University of Ulm where the mHealth application is being developed and used by the patients who have developed tinnitus. \nDr. Ruediger Pryss along with Wilfried Schlee delivered a interesting and inspiring presentation on the application development as its everyday practical use. The presence of patients who are using the application underlined the added value and strengthen the importance of new technologies in the service of health. SUMMARY:JOGG site visit in Helsinki
DESCRIPTION:The event of site visit organized under the auspices of Joint Action Chrodis+ was held on August 13-14th in Helsinki\, Finland\, hosted by THL.  The event provided a platform for deeper approach understanding of the methodology  of JOGG implementation both at local and national level. Selected elements of JOGG will be adapted and implemented within the Health Promoting Community Programme (HPC) in Iceland by the Directorate of Health. \nThe  JA-CHRODIS Good Practice “JOGG” addresses children and adolescent obesity and encourages all people in a town or neighbourhood to make healthy food and physical activity an easy and attractive lifestyle option for young people. HPC is an umbrella for comprehensive health promotion work in municipalities\, including all school levels. HPCs main aim is to support communities/schools to create supportive environments that promote healthy lifestyle\, health and well-being for all. Iceland will adapt selected elements concerning the structure of the JOGG program and the main aim of the HPC will continue to be well being for all. \nThe description of the model project is available here. \nThe objectives of this site visit were: \n\nImproved shared understanding of the JOGG approach\nExchange of good examples from the local implementation and actions from JOGG-cities\nPractical guidance on the local implementation of JOGG\nStakeholder mapping: both national and local – who to include/who do we miss?\nCommunication strategies\nAdvice on how to make contract with the local level\nHow do we support the local programme-managers – the national JOGG-support model.\nDetermine next steps on JOGG implementation
