SUMMARY:Multimorbidity Expert Meeting
DESCRIPTION:The main achievement during 2015 for the JA-CHRODIS work package on multimorbidity was the development of a framework for care of multimorbidity patients that can be applied across Europe. \nFirst\, we identified a list of 20 components across five domains from existing published comprehensive care programs for patients with multiple chronic conditions or frailty. An expert meeting was held in Brussels on October 28\, to discuss the components. Sixteen were selected after discussion. During the meeting the experts discussed the definition of the components\, aims\, key characteristics\, target population and relevance for patients with multimorbidity in order to develop a framework for care of multimorbidity patients\, including recommendations for delivery of system design\, decision support\, self management support\, clinical information systems\, and community resources. \nAn article has been produced to detail the results and conclusions of this meeting\, and has been reviewed\, edited and\, coauthored by all the experts\, entitled “Multimorbidity care model: Recommendations from the consensus meeting of the Joint Action on Chronic Diseases (CHRODIS)”. \nDownloads\nMinutes

DESCRIPTION:The 2015 Conference of Partners of the European Innovation Partnership on Active & Healthy Ageing (EIP on AHA) that will be held in Brussels on the 9th (from 15:00 CET) and 10th December 2015 (all day). This year’s Conference of Partners is dedicated to the theme “Accelerating market uptake of active and healthy ageing innovation across Europe.” \nThe Conference aims at generating an open discussion about the achievements so far and the future vision for the Partnership\, which is now at a crucial stage of its development. \nProgramme of the Conference \nThis year’s Conference of Partners is about focusing on and accelerating delivery towards the 2020 targets. \nIn order to support large-scale deployment of innovative solutions for active and healthy ageing\, the European Commission has launched a number of initiatives. These initiatives are linked to the Scaling up Strategy of the EIP on AHA and it will be discussed how to maximise the impact of some of these opportunities in the course of the Conference. \nRegional and local organisations\, academia\, health and care organisations who have made significant contributions to large-scale deployment of innovation will be invited to present their experiences and share the outcomes of their investments\, in order to recognise and showcase their excellent work in deploying innovative solutions for active and healthy ageing. \nThe revamped EIP on AHA Portal and the Repository of Innovative Practices will be unveiled. External speakers will be invited to explore new synergies with other EU and global initiatives on active and healthy ageing. \nA preliminary programme is available on the website. \nRegistration\nAs in previous years\, the Conference of Partners is by invitation only.
