During the sessions\, panelists and speakers addressed various questions\, such as how the Joint Action can fit with and benefit the EU strategy on chronic diseases\, what can be done to achieve a chronic disease-free Europe\, health systems sustainability and the role of policy makers in addressing the challenge of chronic diseases and ageing society. \n  \nFind all outcomes of the Final Conference here.

SUMMARY:Multimorbidity Expert Meeting
DESCRIPTION:The main achievement during 2015 for the JA-CHRODIS work package on multimorbidity was the development of a framework for care of multimorbidity patients that can be applied across Europe. \nFirst\, we identified a list of 20 components across five domains from existing published comprehensive care programs for patients with multiple chronic conditions or frailty. An expert meeting was held in Brussels on October 28\, to discuss the components. Sixteen were selected after discussion. During the meeting the experts discussed the definition of the components\, aims\, key characteristics\, target population and relevance for patients with multimorbidity in order to develop a framework for care of multimorbidity patients\, including recommendations for delivery of system design\, decision support\, self management support\, clinical information systems\, and community resources. \nAn article has been produced to detail the results and conclusions of this meeting\, and has been reviewed\, edited and\, coauthored by all the experts\, entitled “Multimorbidity care model: Recommendations from the consensus meeting of the Joint Action on Chronic Diseases (CHRODIS)”. \nDownloads\nMinutes

SUMMARY:First WP7 Partners' Meeting

The contribution of the partners was crucial in making the meeting an excellent opportunity to promote exchanges\, discussion\, sharing of resources and experiences. \nThe purpose of the meeting was to present an update of the work package activities\, listen to the partners’ presentations\, and create a melting pot of experiences for a better coordination of work. \nIn particular\, the draft of the questionnaire relative to tasks 1- 4 was presented and discussed. The aim of the questionnaire is to map national/regional strategies on four essential topics: \n\nIdentification of high-risk people\, and prevention of type 2 diabetes;\nSecondary prevention\, including models of care delivery;\nHealth promotion interventions;\nEducational intervention for persons with diabetes\, and training for health professionals.\n\nIn general the questionnaire has been considered useful and complete\, but too long and complex. \nBased on the comments and suggestions raised during the meeting\, a new version of the questionnaire is being developed trying to simplify\, as much as possible\, the structure to allow all the responders to describe their country experiences and contribute to the overview.
